  • Acute insult resulting in diffuse alveolar damage (DAD)
  • Homogenous diffuse ground glass opacities
  • Rapid development, usually intubated at diagnosis

Chest Radiograph
  • Diffuse consolidation, intubated

CT/HRCT

  • Diffuse ground glass opacities to consolidation
  • Central airways may be dilated
  • No pleura fluid or adenopathy
Differential Diagnosis
  • CHF
  • Drug reaction
  • Renal failure
  • Viral pneumonia
  • Diffuse hemorrhage
Pathological Features
  • Unknown insult to alveolar wall
  • Temporally homogeneous
  • Pathologic findings of diffuse alveolar damage
Clinical Presentation
  • Acute fever, cough, SOB rapidly progress to respiratory failure (days)
  • Diagnosis of exclusion, exclude drugs and toxins
  • Supportive care, oxygen, steroids
  • Prognosis poor, mortality 80%
